• A medical invitation letter from an established and well-known hospital. 
• Copies of medical reports and documents regarding the treatment you are undergoing in India. 
• The fee will depend on which nation you are applying from.

• A printed application form signed in blue, as per the signature on the passport, with the uploaded photo.

• Two recent identical photographs of 51mm x 51mm dimensions on a white background.
